Gulf Coast Health Insurance Guide

The Gulf Coast is the most fractured health insurance market in the country. Four of the five Gulf states — Florida, Alabama, Mississippi, and Texas — are among the ten states that still have not expanded Medicaid, while Louisiana expanded in 2016. That single policy split means a household earning $20,000 a year can qualify for free Medicaid coverage in Slidell, Louisiana, yet fall into the coverage gap — too much income for Medicaid, by old rules, but below the marketplace subsidy floor — just across the state line in Picayune, Mississippi. Roughly 1.4 million uninsured Americans sit in that gap across the ten non-expansion states, and a large share of them live along this coast.

The uninsured numbers tell the same story. Per 2024 Census Bureau figures, Texas has the nation’s highest uninsured rate at 16.7%, Florida sits at 10.9%, and Mississippi at 9.7% — all above the 8.2% national average — while Louisiana, the lone expansion state in the region, comes in below it at 7.7%. At the same time, the Gulf states have become the engine of ACA marketplace growth: national enrollment hit a record 24.3 million in 2025, Florida led every state with about 4.7 million marketplace enrollees, and enrollment has more than tripled since 2020 in Texas (up 255%), Mississippi (up 242%), and Louisiana (up 234%).

Layer on a workforce that constantly crosses state lines — Pensacola residents commuting to Mobile, Bay St. Louis shipyard workers living near the Louisiana line, oil and gas crews rotating between Houma, Port Arthur, and offshore platforms — plus seasonal tourism, casino, and construction economies where employer coverage is the exception, and choosing a plan here gets genuinely complicated.
